All 50 States Submit Applications for $50B Rural Health Transformation Program

The Centers for Medicare & Medicaid Services (CMS) has announced that all 50 states have submitted applications for the new $50 billion Rural Health Transformation (RHT) Program. This program was established by the recent Working Families Tax Cuts legislation (Public Law 119-21) to provide funding to strengthen rural health care over five years.

Rural Health Clinics (RHCs) are explicitly listed as eligible to receive support from these funds.

Program Goals and Structure

The RHT Program is designed to help states build more sustainable systems of care. The $50 billion in funding will be allocated from fiscal year 2026 through 2030.

According to CMS, the program's five strategic goals are:

  • Health Innovation: Support rural health innovations and promote preventive care.
  • Sustainable Access: Ensure rural providers can become long-term access points for care.
  • Workforce Development: Build and retain a strong rural health care workforce.
  • Innovative Care: Test new models of care delivery, such as value-based care.
  • Tech Innovation: Expand the use of technologies, like telehealth, that promote access and efficiency.

What Can This Funding Be Used For?

This is the most critical information for RHCs. State plans must commit to using the funds for at least three of the following ten activities:

  • Promoting evidence-based interventions for prevention and chronic disease management.
  • Providing direct payments to health care providers for items or services.
  • Expanding consumer-facing, technology-driven health solutions.
  • Providing training and technical assistance for adopting advanced technologies like AI, robotics, and remote monitoring.
  • Recruiting and retaining a clinical workforce with 5-year service commitments.
  • Providing technical assistance, software, and hardware for IT and cybersecurity upgrades.
  • Assisting rural communities to "right-size" their care delivery (e.g., identifying needed ambulatory, emergency, or post-acute care services).
  • Expanding access to mental health and substance use disorder treatment.
  • Developing projects that support innovative care models, including value-based care and alternative payment models.
  • Other strategies that promote sustainable access to high-quality rural care.

What This Means for RHCs

With all 50 state applications now under review, the timeline is short.

  • CMS Review: Applications are undergoing a baseline and merit review.
  • Awards Announced: CMS will announce the approved state awardees by December 31, 2025.
  • Funding Begins: Funding will be distributed to states beginning in federal fiscal year 2026.
