MIPS 2025 Exception Applications Due December 31st

The deadline to submit MIPS Exception Applications for the 2025 Performance Year is December 31, 2025, at 8:00 p.m. ET.

If you anticipate that you will be unable to report MIPS data fully for 2025, you may be eligible for an exception that could save your practice from a negative payment adjustment.

What Exceptions Are Available?

There are two primary applications available through the Quality Payment Program (QPP) portal right now:

1. Extreme and Uncontrollable Circumstances (EUC) Exception
This application allows you to request reweighting for any or all performance categories.

  • Who is this for? Clinicians, groups, or virtual groups affected by rare events entirely outside their control (e.g., natural disasters, practice closures, or severe financial distress) that prevented data collection.
  • The Result: If approved, the specific categories will be reweighted to 0% of your final score, neutralizing their impact.

2. MIPS Promoting Interoperability (PI) Hardship Exception
This application is specific to the Promoting Interoperability category (formerly Meaningful Use).

  • Who is this for? Practices facing specific technology hurdles, such as:
        • Insufficient internet connectivity (common in some of our rural service areas).
        • Decertified EHR technology.
        • Lack of control over the availability of Certified EHR Technology (CEHRT).
  • The Result: The PI category is reweighted to 0%, and its 25% weight is redistributed to other categories (usually Quality).

Important Details

  • Deadline: December 31, 2025, at 8:00 p.m. ET.
  • How to Apply: You must sign in to the QPP website using your HARP credentials. Navigate to "Exceptions Application" on the left-hand menu.
  • Overrides: Remember, if you are approved for an exception but you still submit data for those categories, your data submission will override the exception. CMS will score the data you submit.

Why This Matters

Submitting an exception application—if you legitimately qualify—is a proactive step to protect your Medicare Part B reimbursements. Ignoring this deadline could result in a penalty in payment year 2027 based on your 2025 performance.
