In a significant move to enhance healthcare access in Mississippi, the House of Representatives has recently passed a bill allowing advanced practice registered nurses (APRNs) to provide patient care without requiring a collaboration agreement with a physician after accumulating 8,000 hours of clinical experience. This legislation addresses the pressing shortage of medical professionals in the state, particularly in rural areas with limited access to healthcare. 

Physician Shortages and Rural Healthcare Access

With Mississippi facing the worst physician shortage in the nation — a situation exacerbated by a significant portion of its physicians nearing retirement — this law comes at a crucial time. State Rep. Samuel Creekmore, chair of the House Public Health Committee, highlighted the bill's objective to increase critical healthcare access. “Most of the healthcare being done in the state of Mississippi is at that mid-level healthcare area, and that’s what this bill addresses,” he stated. 

Key Provisions of House Bill 849

The legislation is set to benefit nurse practitioners, certified nurse specialists, nurse midwives, and certified registered nurse anesthetists (CRNAs). Under the current law, APRNs can provide care aligned with their training without direct supervision but are bound to maintain collaborative agreements with physicians. These agreements require physicians to review a portion of APRNs' treatment charts monthly, adding an unnecessary burden and expense. 

Expanding APRN Autonomy

Under the newly passed HB 849, APRNs who have completed 8,000 hours of clinical practice will no longer need to maintain these collaborative agreements.

CRNA Practice Adjustments

The requirement for CRNAs has been specifically adjusted to ensure they complete 8,000 hours of actual clinical time, underscoring the importance of experience in providing safe patient care. 

Impact on Healthcare Delivery

Enhanced Efficiency and Capacity

Much of the debate surrounding the bill focused on CRNAs, who already provide anesthesia services even when anesthesiologists are absent. As Creekmore noted, the state has around 240 licensed anesthesiologists and approximately 900 CRNAs delivering anesthesia services daily. The new law will empower CRNAs to operate with greater autonomy, ultimately enhancing the overall efficiency and capacity of healthcare delivery in the state. 

Increased Access for Underserved Communities

Many patients in underserved areas already consider advanced practice nurses as their primary healthcare providers. Nurse practitioners often manage clinics that treat common illnesses and chronic conditions without the supervision of a physician. This new law will allow them to practice to the full extent of their training, thereby increasing healthcare options for Mississippi residents.

While House Bill 849 marks a significant step towards enhancing healthcare access in Mississippi, it's important to acknowledge that not all stakeholders are in agreement. Additionally, the bill's passage included an amendment aimed at expanding the Rural Physician Scholarship Program, which will help ensure the future growth of healthcare providers in rural areas. Rep. Bryant Clark's proposal to increase the number of available scholarships from 62 to 100 seeks to address the long-term challenges of physician shortages. 

Legislative Progress

The bill passed the House 75 to 33 and now moves to the Senate for further consideration. Once enacted, this law will streamline the healthcare delivery process in Mississippi and empower nurse practitioners to practice independently, improving patient care across the state.
